All education verification requests must be accompanied with a completed copy of the York Adams Academy Release of Student Records Form. A copy of the form can be found below. The completed form should be submitted via email to the Administrative Assistant to the Director (firstname.lastname@example.org). If you do not have email, the completed form may be faxed to the attention of Melanie Meakin to 717-767-4336. Once the completed form is submitted, York Adams Academy will be happy to verify the request. The Academy will not process any educational verification requests without prior completion of the Release of Students Records Form. It is the responsibility of all graduates to obtain and submit this form prior to the education verification request being made.
York Adams Academy graduates (2009-Present), should contact their home school district's guidance office to inquire about the transcript acquisition process.
York County High School graduates (1990-2008) please complete the form below and submit it via email to the Administrative Assistant to the Director (email@example.com).
